Self care for long-term conditions

Self care is about supporting people to help themselves to manage commonly occurring conditions and long term illness.
For example, an average person with diabetes spends 6 hours a year face-to-face with a health professional. It's the decisions that they make in the other 8,754 hours that will determine the quality of their life.
Self-care week, (14-20 November) is supported by the self care forum and this year aims to highlight how technology can patients confidently manage illness. That's why we have compiled useful online resources that we think will be helpful for anyone diagnosed with a long term condition.
